Private and Professional (non-nhs services) - Fees April 2009

Your doctor will normally treat you under the NHS. However your doctor is entitled to charge you for some services.

Non-NHS services which may be charged for include:

• Access to records
• Accident and sickness insurance medicals and reports
• Certificates—such as sick notes for the first week of work absence
• Drivers’, school and sports medicals
• Holiday insurance certificates and passport applications
• Private health insurance claims
• Private prescriptions for travel abroad
• Some vaccinations for travel abroad

In the event that a form is required to be completed, the fee will be payable on leaving the form at reception. You will be provided with a receipt. Completed forms will be posted back to your home address within 21 working days of the receipt date. 

Please note, this may take longer if the form can only be completed by a specific doctor and that doctor is away from the practice.  

If your completed form has not been returned to you within 28 working days, please telephone the surgery (024) 76366775 and ask for the Administration Department.
