Register as a new patient

If you live within the practice area you are entitled to join our practice list. We do not discriminate against any patient wishing to join our list. Ask a receptionist for a registration form (GMS1). A form should be filled in for each member of your family. If completed online you will also need to call the surgery to book an appointment to arrange a new patient medical with a nurse.

If you change your details

Please inform the reception if you change your name or address. There is a form to fill in and it is useful to include your telephone number and postcode. If your new home is out of the practice boundary, we will unfortunately have to ask you to re-register with another practice nearer to your new home.
